Privacy Policy

We respect your privacy and take all necessary steps  to ensure your care is kept confidential.  The Federal government has enacted certain laws to protect the confidentiality of your health information. These laws are written under the Health Insurance Portability and Accountabillity Act - commonly referred to as H.I.P.A.A.

We will communicate your Health Information only for the  purpose of:

1. Providing Treatment
2. Obtaining Payment
3. Conducting Health Care Operations 

Your information will not be used for other purposes unless we have asked for and been voluntarily given your written permission.

You will be given a copy of our Notice of Privacy Practices Brochure during your initial visit to our office.  In addition, patients may request a copy of our complete Privacy Policy at any time by contacting our Privacy Official at #571-2020.
